In the event of a last minute or same day cancelation, there will be a $35.00 fee assessed, at your next visit, if you fail to provide a 24 hour cancelation notice to me.  I will handle these occasions on a case by case basis. I am reachable via voice, email or text. Text is the fastest response as I do NOT have a personal assistant answering calls. I have an automated booking system that sends out a text 48 hours prior to your appointment. You will then have an opportunity to cancel your appointment.

If you are using the Therapeutic Massage benefit of your health insurance plan, you MUST present a Choose Healthy voucher at your appointment or discount will NOT be honored. Please bring your insurance ID card for verification purposes. Choose Healthy vouchers can ONLY be used by the person named on the ID card.

Steps to print the Choose Healthy Voucher:

1. Visit www.choosehealthy.com. You may need to create a separate login/password.

2. Click on Find a Provider

3. Select Therapeutic Massage

4. Click on location. You can search by zip code 85225. Select Chandler, Az

5. Select Balanced Therapeutic Bodyworks.

This will take you to a page where you can print the discount voucher. This MUST be done for every session that is booked with me. In the comments section, let me know you are using the Choose Healthy Voucher. 

For new clients, please arrive 10 minutes prior to your session to fill out the required intake forms or you may print them out and bring it to your session. Visit the forms area on the main menu.
